  • Lead the implementation efforts to successfully install the new application (inclusive of system

assessment and planning, requirements gathering, and any system customizations required).

  • Support the development and execution of the data migration strategy.
  • Develop a comprehensive change management strategy.
  • Support the system testing plan.
  • Support post-implementation stabilization and operationalization.

Our objective is to select a Jiva™ implementation consultant with strong expertise (preferably Jiva™

certified) in the specific platform, and with the relevant background experience to successfully execute and

support successfully meeting the implementation deadline.

The project will require a consultant that works with a keen sense of urgency, is a well-organized and

skilled planner, and possesses excellent communication skills.

  • System assessment and planning: Assess the existing care management processes (currently

managed within Essette), identify gaps, and design a plan for system implementation.

  • Requirements gathering: Collaborate with the health plan to define their care management system

requirements, including functionalities, workflows, and reporting capabilities.

  • System customization: Tailor the ZeOmega platform to align with the health plan's unique

workflows and processes. [Goal is to keep as close to the out-of-the-box configuration as possible while

ensuring compliance and no loss of the current system’s functionality]

  • Data migration: Ensure a smooth transition of data from legacy systems to the ZeOmega platform,

including data cleansing, standardization, mapping, and validation. [Provide a data migration strategy and

plan]

  • Integration with existing systems: Direct the integration of the ZeOmega platform with other

existing systems (e.g., electronic health records, claims systems (HSP), inclusive of proprietary and third[1]party systems.) to facilitate seamless data exchange and interoperability.

  • associated with adopting a new care management system, including stakeholder engagement and

communication strategies. Support stakeholder decision-making and negotiation.

  • System testing and quality assurance: Ensure ZeOmega completes thorough testing of the care

management system to ensure its functionality, reliability, and security. This includes establishing a

comprehensive quality plan, as well as managing that plan.

  • Post-implementation Stabilization and Optimization: Conduct assessments and collect feedback

after the system implementation to identify areas for improvement and optimize system performance.

Job Type: Contract

Pay: $60.00 - $100.00 per hour
